The current financial resources and recent changes imposed by the State in the relationship with sponsors and pharmaceutical companies have further weakened the training system. The lack of dissection labs and simulators forces young neurosurgeons to undergo frequent, expensive internships abroad, which are often self-funded.
The Needs
International competitiveness makes a change in direction necessary. To train a neurosurgeon capable of handling complex approaches, such as those for skull base tumors or vascular pathologies, a structured and planned training program is essential, much like the preparation of an airplane pilot. This comparison is not coincidental, as there are many similarities. In all professions where there is zero tolerance for error, it is necessary to subject the specialist to an intensive training period, preparing the neurosurgeon for all possible situations, just as a pilot is trained to handle adverse weather conditions or sudden malfunctions.
Books are fundamental, but they are not enough.
They are necessary for the training of the neurosurgeon:
- Anatomical dissections, which are currently irreplaceable and make it possible to achieve real knowledge of three-dimensional anatomy;
- Simulations using increasingly realistic software make it possible to create different situations with which one has to deal. Simulators also allow measurement; they are therefore qualitative and quantitative indicators that make it possible to follow and monitor the improvement of the specialist.
All this has important costs.
In particular, the following critical points must be taken into account:
- Unlike clinical research, neurosurgeon training does not have access to international funding and donations.
- The costs of the daily use of laboratories and simulators and the implementation of projects are not sustainable without external help.
